Hi, I just join as a new member when I discover this chat site. I had MM in early 2001. After chemo treatment with VAD, I had auto transplant in 2001 Oct. About 1.5 years later I have CR, but I still took Thailidomide of 200mg for 4 years until 2007. Then the thailidomide was reduced to 150mg and for the whole year of 2007 I was not followed up for MM. In early 2008 when I returned to follow up, my MM had relapsed. Then I had Velcade for 16 pins together with Dx and Thailidomide. I did not achieve CR. Thereafter, 6 months after completion of Velcade my Igg went up to 2500, and I had to take another 16 pins of Velcade up to now. I took routine 200mg of thailidomide per day since 2008.
Seems that Velcade is not so effective for me. Can anyone advise me that if I take a second auto BMT(my brothers and sister's bine marrow do not match mine), will it be feasible so that drugs(vecade, thailidomide, Dx.ect) may be more effective, or fortunately I can even achieve CR? But this auto BMT will be 8 years after my first one.

Hi,
If we know your platelet count at this time, we can give you a better idea of whether or not you will be able to collect more stem cells to make a transplant possible. The higher the platelet count, the more likely it is that you can collect stem cells again.
As far as whether or not STC is the best option for you, our myeloma specialists can not give their opinion on that without first seeing you in the clinic to examine you and your medical history. But the thing to look into is whether or not stem cell collection is an option for you, and then you can go from there.

Dear Chat Moderator,many thanks for your reply.My platelet count is always 200+ except at the time when I'm having velcade treatment. Below is my recent data.

2008 Dec end 492 2550 11.6
(before velcade
injection)
2009 Jan end 214 1660 9.9
2009 Feb end 190 1460 8.6
2009 Mar end 166 - -
2009 Apr end 265 1560 7.5
(after velcade
injection of 16 pins)
From the data I think velcade is not so effective for me, that's why I think of a 2nd BMT.It has no relationship with financial problem.
Some more information of myself:-
I'm now 60 years of age, I enjoy CR(paraprotein=0)from 2004 Jan to 2007 july(estimate, because I did not follow up within 2007)
I have been taking 200mg of thailidomide since end of 2002 up to now except the whole year of 2007, which I took 150mg.The side effects are acceptable to me. Can I increase it to 250mg?

Hi,
Thanks for the info. With such a good platelet count, it would appear that stem cell collection would very likely be possible for you.
I have spoken to one of our doctors about your thalidomide question and he says that, in his opinion, he would not increase the thalidomide. It is unlikely someone will respond "better" to 250 mg than 200 mg. Plus, the higher dose is more toxic, so there would seem to not be an advantage but rather a disadvantage. Our doctors generally recommend not taking more than 100 mg of thalidomide per day.
